* Due to the new restrictions during this pandemic, to protect everyone's health, and to ensure the quality of your service, we kindly ask all client to:
- Change or Cancellation must be made 24 hours prior to appointment time to avoid a 50% service Fee.
- Please confirm your appointment by replying to our text messages.
- Arrive no later than 5 minutes for your appointment, otherwise your appointment will be considered as walk-in status.
- Wear a face mask when coming in for service.
- Not bringing in small children or addition guest who is not receiving service.
- Thoroughly washing your hands before service.

I understand that:
- Contact with others during this worldwide Coronavirus (COVID-19) including but not limited to salon staff and other salon patrons involves a certain degree of risk that could result in illness, permanent injury, or death.
- I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it and who does not given the current limits in virus testing.
- Anthony Vince’ Nail Spa assumes no responsibility for any medical care that I incur.
I confirm that:
- I am not presenting any of the following symptoms: Fever, Shortness of Breath, Loss of Sense, Taste or Smell, Dry Cough, Runny Nose, or Sore Throat.
- I have not been in the presence any COVID-19 positive person within the last two weeks.
- In consideration of the permission to participate in the Activity, I agree to the terms contained in this document.
1. ASSUMPTION OF THE RISKS: I hereby freely assume the above-mentioned risks as well as other risks not listed that are part of this salon service, and any harm, injury or loss that may occur to me or my property as a result of my participation in the salon service—including any injury or loss caused by the negligence of Anthony Vince’ Nail Spa, its employees, agents and officers, its contractors, and other staff.
2. RELEASE OF LIABILITY: I hereby RELEASE Anthony Vince Nail Spa, its employees, agents, officers, and contractors, and the providers of any service in the Salon, (“the Released Parties”) FROM ALL LIABILITIES, CAUSES OF ACTION, CLAIMS AND DEMANDS that arise in any way from any injury, death, loss or harm that occur to me from the salon service or in any way related to the salon service and all claims and liability resulting from exposure to disease-causing organisms and objects, such as COVID-19. This RELEASE includes claims for the negligence of the Released Parties and claims for all liability of the rendered salon service. I also agree NOT TO SUE or make a claim against the Released Parties for death, injuries, loss or harm that occur from the salon service or are related in any way to the salon service.
3. INDEMNIFICATION HOLD HARMLESS AND DEFENSE: I promise to INDEMNIFY, HOLD HARMLESS AND DEFEND the Released Parties (defined in Section 2) against any and all claims to which Section 2 of this agreement applies, including claims for their own negligence. In accordance with these promises, I will reimburse the Released Parties for any damages, reasonable settlements and defense costs, including attorney’s fees, that they incur because of any such claims made against them.
HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS AGREEMENT BY READING IT. NO ORAL REPRESENTATIONS, STATEMENTS, OR OTHER INDUCEMENTS TO ACCEPT THIS RELEASE HAVE BEEN MADE APART FROM WHAT IS CONTAINED IN THIS DOCUMENT. I UNDERSTAND THIS IS A CONTRACT THAT AFFECTS MY LEGAL RIGHTS AND I ACCEPT IT OF MY OWN FREE WILL.
